Transition Discoveries


Planning for LIFE After High School

young adults working together in classroom settingThe Transition Discoveries project is hosting a series of focus groups for young persons with disabilities, and their families, who are planning for the transition to life after high school.

The project is inviting individuals from across the Commonwealth to make a difference by sharing their stories!

Researchers are anxious to hear the authentic voices of youth with disabilities and families about their experiences in planning for life after high school. The information gathered at the focus groups will help researchers design a survey to find out what is happening with youth during this important time in their lives and in the lives of their family.

The final data collected will be used to have an impact on systems throughout Pennsylvania, thereby enhancing the lives of young individuals with disabilities and their families. This project runs through June 2017.

What to expect?

Two different focus groups at each of eight locations: one for families; one for youth with disabilities.

Participants will be encouraged to share stories: plans after high school, hopes and dreams, strengths, talents and gifts, and fears and challenges.

In addition, there will be a unique opportunity to network with others.

Who is invited to apply?

Youth with disabilities (ages 14-24).

Family members of youth with disabilities (ages 14-24).

Selection to participate will be based on cross-disability, cultural diversity, and statewide representation.
